NHS Charities Together NHS Charities Together are pleased to announce the funding made available to Integrated Care Systems. For this area that is Humber Coast And Vale and stretches along the east coast of England from Scarborough to Cleethorpes, and along both banks of the Humber. Humber Coast and Vale incorporates the cities of Hull and York and Large and rural areas of East Yorkshire, North Yorkshire and Northern Lincolnshire.
These grants are to support community partnerships in recognition of the impact of COVID-19 on the wider community.
As the lead charity for this project, Health Tree Foundation have worked with the NHS Charities in the area to agree three funding pots for the voluntary and community sector to apply. The pots are outlined below.
Pot 1: £200,000 for Green Social Prescribing: Mental & Physical health
This funding pot will be aligned with the HCV Green Social Prescribing Bid. Preventing admission to NHS facilities, developing early and or alternative interventions to clinical need.
For this fund we will accept applications for up to £20,000 to apply for this funding pot you will need:

Pot 2: £200,000 for Volunteering: 2 bids sought 1 from Humber area, 1 from York & North Yorkshire
£200,000 split between two projects: One in the Humber area and one in York &North Yorkshire. Programmes sought that show learning and new investment into volunteering to capture community spirt, helping to maximise the role of volunteers in your communities to:
Prevent admission to NHS facilities, developing early and or alternative interventions to clinical need.
Facilitating discharge from NHS facilities, for example with end of life care or community health projects
Supporting patients to recover more quickly in the community after leaving hospital
Supporting initiatives that seek to remove health inequalities and disparity in health outcomes with a focus on diversity in the population
Applicants must clearly evidence that they are an organisation that specialises in leading and supporting volunteering projects within one from the Humber (Hull, East York’s, North Lincolnshire, North East Lincolnshire) area and one from the York & North Yorkshire; to provide a proposal which outlines how with investment; you can implement lessons learned from the past years volunteer experiences for example:

Pot 3: £200k for Match funding projects grants from £5,000 to £50,000
You must already have raised or committed funds for your project and we will match fund up to £50k for a minimum of four projects that meet one of the following outcomes:
